Practice Boundary

The Practice boundary covers an area of 10 miles radius from the surgery.

Registration Of New Patients

All newly-registered patients will be asked to complete a simple questionnaire.

There are 2 registration forms for different circumstances in NI:

  1. Patients wishing to transfer GP within Northern Ireland or transferring from Great Britain need to complete a form HS200.
  2. Eligible visitors or persons taking up ordinary residence in Northern Ireland need to complete a form HSCR-1.  
  3. We also require new patients to fill out a simple health history questionnaire.
  4. Eligibility can be confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.

If you are staying in Portrush you may register with us temporarily. If you require any of our services, please discuss this with the receptionist.

Updating Your Details

If you move or change any of your personal details (e.g. address, surname, phone number) please help us by informing us so that your records can be updated. 

Failure to do so could result in delays or prevent us from contacting you or a relative in an emergency.

Disabled Patient Facilities

We make every effort to make the surgery accessible for disabled patients.  There is access through the main door and all consultation rooms are on ground level, the surgery entrance and toilets are suitable for wheelchair access.

We have disabled access for all patients services and facilities.

The public car park in front of the surgery contains designated disability parking spaces.
